Job summary

Hillview Health Care Center is seeking a compassionate Licensed Practical Nurse (LPN) for the PM shift to deliver high-quality nursing care in a resident-centered environment.

You will administer medications, observe residents, document changes, and assist in developing individualized care plans under RN supervision. A Wisconsin LPN license and background check are required, with preference for long-term care experience and CNA supervision.

Qualifications

• Graduation from an accredited nursing program
• Current Wisconsin LPN license
• Experience in long-term care preferred
• Ability to supervise Nursing Assistants
• Strong clinical skills, compassion and resident-centered care
• Must pass Wisconsin Caregiver Background Check
• Valid Wisconsin driver’s license if driving is required

Responsibilities

• Provide high-quality nursing care under the supervision of a Registered Nurse
• Administer medications and treatments in accordance with the Nurse Practice Act
• Observe residents, document care, and report changes in condition
• Assist in developing and implementing individualized care plans
• Support training and orientation for new staff
• Ensure a safe environment for residents and staff
• Uphold residents’ rights, privacy, and person-centered care principles
• Create a positive, respectful, and compassionate experience for residents, families, and colleagues
