Job summary

Phoenix Rescue Mission in Phoenix, AZ is seeking a compassionate case management specialist to support RAP Program clients with dignity and respect.

You will conduct phone screens and intake, gather required documentation, schedule care, coordinate referrals, and maintain accurate data in the Mission's database.

Qualifications

• High school diploma or GED required; Bachelor's degree preferred in a related social service field.
• Minimum of one year of experience in social services, intake, behavioral health, customer service, or related field.
• Experience in intake, scheduling, and coordination of care is strongly preferred.
• Certification in Peer Support is highly desired.
• Proficient in Microsoft Office; program database experience preferred.

Responsibilities

• Conduct phone screens to determine prospective clients' eligibility for the RAP Program and gather detailed information for appropriate placement.
• Complete intake appointments, obtain required documentation, releases, and consent forms, and ensure accurate data entry into the Mission's database.
• Schedule case management appointments and follow-up services to support smooth transitions into care.
• Conduct property searches and urine analyses per program policies and procedures.
• Connect clients with community resources and coordinate external referrals when eligibility or levels of care differ.
• De-escalate crisis situations and communicate effectively with clients, families, and team members to promote positive outcomes.
• Build collaborative relationships with community providers and internal teams to ensure continuity of care.
• Adhere to all legal, regulatory, and compliance requirements and to the Mission's policies and Code of Ethical Conduct.
• Follow the Mission’s Statement of Faith and related policies; participate in faith-based activities when appropriate.
• Lead or participate in devotions or other organizational activities that promote a credible Christian witness and lifestyle.
• Perform other duties within scope as assigned.
