Penn Medicine is seeking an Evening-Shift Inpatient Pharmacist to join the Central Pharmacy, OR Satellites, and IV Room teams. Responsibilities include order verification, dispensing medications, and providing clinical guidance to ensure safe drug therapy.

Minimum requirements include Pennsylvania licensure and a Pharm.D or BS in Pharmacy. This full-time role follows rotating evening shifts with weekend/holiday requirements in a renowned academic medical center.

Qualificações

• Licensure from the Pennsylvania State Board of Pharmacy required before start date.
• Pharm. D or Bachelors in Science in Pharmacy.

Responsabilidades

• MAINTAIN PATIENT PROFILES; report unresolved problems to Clinical Coordinator for further research and clarification.
• PREPARES AND DISPENSES MEDICATIONS USING APPROPRIATE TECHNIQUES; send initial supply to floor per schedule.
• DISPENSE MEDICATION TO OUTPATIENTS AND NURSING HOME PATIENTS; maintain accurate records.
• SERVE AS A RESOURCE FOR MEDICAL AND NURSING STAFF; answer drug information questions.
• ENSURES CONTINUITY OF PHARMACEUTICAL CARE TO AND FROM ACUTE AND AMBULATORY CARE SETTINGS.
• PHARMACEUTICAL CARE: review medication history, educate patients, develop plans, monitor outcomes.
• COMPLEX IN ALL ACTIVITIES WITH ACCREDITATION, LEGAL, REGULATORY, AND SAFETY REQUIREMENTS.
• COMPLIES DRUG RELATED PROBLEMS.
