Zusammenfassung

MyMichigan Health is seeking an on-call Staff Registered Nurse to provide skilled home care nursing within the agency's service area from 4:00 PM to 9:00 PM, Monday through Friday, with no weekend or holiday rotation. Orientation requires full-time hours for 2–4 weeks based on prior home care experience.

The RN will perform assessments, develop and adjust plans of care, and coordinate with LPNs and home health aides to ensure quality patient outcomes.

Qualifikationen

• One year of direct patient care experience is preferred.
• Orientation requires the ability to work full-time hours for 2 to 4 weeks, depending on prior home care experience.
• RN license and active state certification required (home health).

Aufgaben

• Regularly assesses patient’s needs and provides skilled nursing services as prescribed by the physician authorized plan of care.
• Initiates plan of care for assigned patients and revises and adapts as necessary.
• Provides nursing care (or arranges for services through specialized staff) through ongoing visits to a group of patients, sharing the patient caseload.
• Coordinates patient care by evaluating outcomes and providing ongoing feedback to others involved in care relative to patient status and changes needed on the care plan.
• Coordinates care by supervising LPNs and Home Health Aides; supervises LPNs as needed.
• Notifies physician of changes in patient condition and obtains new orders as needed.
• Implements preventative and rehabilitative nursing procedures; maintains patient records and supports agency policies.
