Qualifications

• High School Diploma or equivalent; Must be at least 21 years old.
• Valid driver’s license required.

Responsibilities

• Assist with activities of daily living and personal care.
• Help with light housekeeping related to health and well-being.
• Assist with accessing community resources, shopping, and appointments.
• Complete Daily Activity Logs and Contact Summaries promptly.
• Adhere to safe transportation procedures.
• Follow protocol for I/A and Serious Incident reports.
• Maintain flexibility to accommodate family needs and schedule.
• Follow Activity Plan and family instructions; update supervisor with pertinent information.
• Follow safety policies and worker's compensation guidelines.

Education
