Job summary

Qualifications

• Bachelor’s degree in Human Services or related field and a valid driver’s license with acceptable driving record.
• Johnson County Government requires reference/background screening for all positions.
• Education/Experience substitutions may apply; experience can substitute for degree or vice versa.

Responsibilities

• Conducts assessments of individuals to identify needs and eligibility; communicates information to individuals, families, and stakeholders.
• Provides coaching and support to family members to create a safe home and community environment.
• Assist families in identifying healthy coping strategies to reduce caregiver strain and improve relationships.
• Develops plans addressing needs across multiple areas of focus; coordinates with community agencies for resources.
• Completes and submits documentation; tracks progress against measurable goals; evaluates service effectiveness.
• Supports emergency readiness and cross-departmental collaboration as needed.
