Job summary

Journey Found, Inc. in Manchester, CT is seeking a Direct Support Specialist to assist individuals with daily living tasks in our group homes.

You will support activities of daily living, transport clients, organize community outings, and monitor health with medication administration under supervision. The role requires compassionate, patient care; you will document program data, communicate with nurses and supervisors, and work as part of a team.

Qualifications

• Requires a high school diploma or equivalent.
• 3+ years driving with a valid license.
• Behavioral management experience or formal training.
• Excellent oral and written communication skills.
• Willingness to obtain PMT, CPR, FA and DDS MAP training.

Responsibilities

• Assist individuals with activities of daily living (dressing, cooking, cleaning, bathing, toileting).
• Transport clients to appointments and activities; organize community outings.
• Provide medication administration support and accompany to doctors’ appointments.
• Document program data and maintain accurate records; teach and engage residents.
• Implement routines and assist in decision-making; run errands including grocery shopping.
• Deescalate situations, manage behaviors, and communicate professionally with Nurses and Supervisors.
