Job summary

Rochester Regional Health is seeking a Certified Nursing Assistant (CNA) for Full Time Evening shifts at Clifton Springs Nursing Home. The role supports short-term rehab and long-term care residents with essential daily care.

You will collaborate with a multidisciplinary team in a hospital-connected rehab unit to ensure smooth care transitions. Qualifications include NYS CNA certification and a high school diploma or equivalent.

Qualifications

• Minimum age 18
• CNA certification in New York State
• High School diploma or equivalent

Responsibilities

• Read and follow care cards for assigned residents and coordinate with nurses for care plans.
• Provide personal care including bathing, toileting, feeding, transferring and mobility assistance.
• Assist with personal services and room upkeep, and help visitors as needed.
• Report resident status changes and hazardous conditions to the nurse on duty.
• Observe and support residents in daily living activities and participate in care conferences.
