Job summary

• Master's degree in Social Work required.

Responsibilities

• Provide therapeutic intervention and social work services to patients and families to enhance comprehensive, integrated, and uninterrupted care in the hospital and to facilitate continuity of care in the community.
• Demonstrate positive interpersonal communication with patients, families, visitors, and co-workers according to policies.
• Coordinate services and advocate to remove barriers to post-acute care and community resources.
