Job summary

TPC Wellness in Ironton, OH seeks a compassionate Case Manager for a part-time evening schedule (Mon-Fri 4pm-8pm). You will manage caseloads, coordinate resources, and document in our EHR under supervision of the Clinical Supervisor.

The ideal candidate holds a CDCA certification or can obtain within 30 days, possesses a high school diploma, and has strong organizational, communication, and crisis-management skills to support clients on their recovery journey.

Qualifications

• CDCA certification or ability to obtain within 30 days of hire.

Responsibilities

• Maintain assigned caseloads and ensure regular client contact.
• Monitor client progress and assess ongoing needs per policies and contracts.
• Meet with clients individually and in group settings.
• Collaborate with families, clinicians, case managers, and community partners.
• Provide crisis intervention, advocacy, and resource coordination.
