Job summary

The Experience Senior Living Team is seeking a Care Partner to join our dedicated team in Fort Collins. This role offers a 2pm-10pm shift, full-time status, and a hourly compensation of $19-$21.

You will assist residents with personal care, daily living activities, and participate in life enrichment programs, while maintaining infection control and documenting reports. We provide a full benefits package and emphasize compassionate, customer-focused service to residents and their families.

Qualifications

• High School Diploma or equivalent.
• Certified Nursing Assistant/Home Health Aide preferred, but not required.
• License/Certificate for Medication Assistance (if applicable) per state regulations preferred.
• CPR certification/First Aid certification preferred.
• Must complete all state and Experience Senior Living specified training programs.

Responsibilities

• Assist residents with personal care and activities of daily living as assigned and per service plan.
• Perform daily housekeeping tasks according to established procedures and standards.
• Perform laundry duties as assigned.
• Review and participate in 24 Hour Reports, shift change reports and verbal reports.
• Assist residents to and from scheduled life enrichment programs.
• Promote personal choices, independence and spiritual needs as needed.
• Provide verbal encouragement and support to residents.
• Prioritize tasks to ensure optimum services to residents as needs change.
• Communicate changes in resident needs to the Director of Health & Wellness and other departments as needed.
• Respond to resident pendant/wearable calls promptly.
• Ensure compliance with new resident care needs and infection control practices.
• Report accidents and incidents to supervisor.
• Maintain confidentiality of residents' personal care information.
• Honor residents' personal and property rights.
• Be prepared for emergencies including disasters, fire, and other emergencies.
