Job summary

Monarch Communities in Connecticut is seeking a Care Manager (CNA) for a full‑time evening shift (2:30 pm–10:30 pm). The role involves assisting residents with daily living activities, following care plans, and promoting safety, comfort, dignity, and well‑being.

You will report to the Health & Wellness Director. Duties include documenting services, escorting residents to meals and activities, and promptly reporting changes in condition.

Qualifications

• Certified Nursing Assistant or Certified Home Health Aide required.
• Experience working with elderly residents preferred, including dementia exposure.
• Ability to lift up to 50 lbs and perform personal care tasks.

Responsibilities

• Provide resident care including bathing, dressing, toileting, and feeding per care plan.
• Maintain safety, dignity, and confidentiality of residents.
• Escort residents to meals and activities.
• Document all services provided during the shift.
• Report any changes in condition to the supervisor.
• Knowledge of dementia care preferred.
• Report changes in condition to supervisor promptly.
