Job summary

Edward Elmhurst Health in Warrenville, IL seeks a Part Time Patient Navigator for the Behavioral Health department on the evening shift (3:00pm-11:30pm). 20 hours/week with rotating weekends/holidays.

Responsibilities include initial client contact, assessments, crisis triage, and insurance verification. Qualifications require an Associate’s Degree plus 3 years of experience or a Bachelor’s Degree; CPI and AHA CPR certifications within 90 days.

Qualifications

• Education: Associate’s Degree plus 3 years of experience or Bachelor’s Degree.
• Preferred: Bachelor’s in Behavioral Health.
• Certification: CPI within 90 days; CPR certification within 90 days.

Responsibilities

• Responds to and triages crisis calls.
• Registers and reviews patient information in the system.
• Schedules mental health appointments and conducts initial assessments.
• Verifies insurance coverage and eligibility for Blue Cross HMOI members.
