Job summary

Qualifications

Responsibilities

• Develop and evaluate plan of care.
• Systematically assess patient knowledge and health through interviews and exams.
• Consult with multidisciplinary care team on assessment findings.
• Create individualized plan of care based on patient problems.
• Communicate changes in patient status to team members.
• Provide and delegate patient care per protocols.
• Conduct rounds, administer medications, and document care.
• Identify learning needs and educate patients and families.
• Participate in Quality Improvement and staff meetings.
