Job summary

UMass Memorial Health is seeking a Point of Entry/Emergency Department Case Manager to coordinate and facilitate care for ED patients, both inpatient and observation, using InterQual criteria. The role emphasizes timely evaluations, discharge planning and collaboration with MSW and ED teams.

The CM will manage initial discharge needs, refer complex cases, and document utilization reviews while ensuring smooth transitions and escalation when needed.

Qualifications

• Bachelor of Science in Nursing required.
• Current Massachusetts RN license required.
• 3+ years medical, surgical, ICU, or ED nursing experience preferred.
• Excellent interpersonal and communication skills required.
• Analytical ability to evaluate patient care needs and services.

Responsibilities

• Coordinate and facilitate care for ED patients, inpatient and observation.
• Review ED patients promptly and within one hour of admission when possible.
• Collaborate with MSW, case management, and ED team for discharge planning.
• Complete discharge planning assessments and arrange post-acute services.
• Escalate unresolved cases and document delays for boarded ED patients.
