Job summary

Northwell Health is seeking a registered nurse to perform age-specific care using the nursing process across a designated patient group. The role requires leadership, collaboration with physicians and the health team, and delegating activities on the patient care unit.

The nurse will assess physiologic and psychosocial health, identify outcomes, formulate and implement a care plan, and evaluate care effectiveness with attention to discharge planning and family education.

Qualifications

• Graduate from an accredited School of Nursing.
• Current license to practice as a Registered Professional Nurse in New York State.
• BSN preferred.
• 2 years of Med/Surg, Telemetry Experience, OR ED Experience Highly Preferred

Responsibilities

• Assesses the patient's physiologic health status and documents findings.
• Assesses patient's psychosocial health status and coping abilities.
• Identifies patient outcomes and develops individualized goals.
• Implements the plan of care and coordinates resources for safe care.
• Evaluates care effectiveness and participates in discharge planning and referrals.
