Job summary

You will assess physiologic and psychosocial health, determine outcomes, plan and implement care, and evaluate results while guiding the care team and ensuring safety and quality for patients.

Qualifications

• Graduate from an accredited School of Nursing.
• Current license to practice as a Registered Professional Nurse in New York State.
• BCLS, ACLS, preferred.

Responsibilities

• Assesses the patient’s physiologic health status and documents plan of care.
• Assesses patient’s psychosocial health status and identifies needs for support services.
• Identifies patient outcomes and establishes individualized goals.
• Formulates the plan of care and coordinates resources to achieve outcomes.
• Implements the plan of care with safe, effective treatments and accurate documentation.
• Evaluates care and outcomes, adjusting as needed to improve quality.
