Job summary

Qualifications

Responsibilities

• Develops and implements a care plan involving patient and family.
• Delivers direct patient and family centered care and education per standards.
• Identifies and responds to patient conditions to achieve positive outcomes.
• Performs discharge planning for safe transition of care.
• Directs and delegates care provided by unlicensed team members.
• Provides world class customer service to patients and families.
