Qualifications

• Experience with discharge planning and patient education.

Responsibilities

• Assess and monitor patients; perform initial and ongoing assessments according to policy.
• Plan and coordinate patient care with patient, family, and health care team.
• Communicate plan of care during shift handoffs and daily conferences.
• Direct care team members in accordance with scope of practice.
• Document patient care, plan of care, interventions, and physician orders; provide education.
• Evaluate outcomes and involve patient/family in care planning.
