You will lead a multidisciplinary team, evaluate medical necessity, and align care with hospital priorities while managing resource use.

The role requires one year of inpatient nursing experience and a valid RN license, with strong communication and collaboration skills to support patients and families during urgent ER care

Qualifications

• One year of nursing experience in an inpatient setting.
• RN license required.

Responsibilities

• Performs and documents comprehensive assessments for ER patients requiring case management.
• Collaborates with patients, families, and the interdisciplinary team to plan care in urgent ER settings.
• Continuously reassesses and documents patients’ needs and effectiveness of interventions.
• Facilitates timely access to resources, coordinating with hospital departments and social services.
• Coordinates case management interventions to meet ER timeframes.
• Leads case management activities for high-risk ER patient populations.
• Monitors patient progress and adjusts care plans as conditions evolve.
• Evaluates appropriateness and medical necessity of proposed care plans.
• Facilitates safe transitions, including admissions, transfers, or discharge with follow-up.
• Collaborates with social workers, nurses, physicians, and teams for immediate and long-term needs.
• Uses reports and data to evaluate performance and support quality improvement.
