Job summary

OU Health is seeking an Adult Care Management RN for the Emergency Department on a full-time alternating schedule. You will assess patients, develop and coordinate care plans, and educate patients and families while ensuring high-quality outcomes.

Qualifications

• RN license in OK or eNLC (multistate)
• Basic Life Support (BLS) required by the American Heart Association
• Level 1 requires 0–3 years nursing experience; Level 2 requires 3+ years care management; Level 3 requires 5+ years care management
• Education requirements vary by level (AA, BSN) as stated in the posting

Responsibilities

• Conduct comprehensive assessments of patients' health status, medical history, and ongoing care needs.
• Develop individualized care plans with the care team, patients, and families to ensure continuity of care.
• Provide education to patients and families about health conditions, treatment plans, medications, and self-care.
• Coordinate communication between patients, families, providers, and resources to optimize outcomes.
• Evaluate effectiveness of care plans and adjust as necessary.
• Evaluate utilization patterns and identify opportunities for cost-effective care.
• Advocate for appropriate resources and ensure regulatory compliance and reimbursement criteria.
• Collaborate with insurers, social workers, and other stakeholders for timely authorizations.
• Monitor patient outcomes and care delivery processes to identify improvements.
• Participate in quality improvement initiatives and interdisciplinary conferences for patient safety.
