Job Summary
The Care Coach is a nationally certified medical assistant who works directly with patients in one-to-one encounters and group sessions to reinforce self-management of their chronic condition while closely monitoring their established care plan goals.

Physical Demands

The employee will be frequently required to reach with hands and arms, bend, balance, kneel, crouch, crawl, push, and pull.
