• Must have CASAC Level 1 Certification or equivalent.
• Must have completed OASAS MAT training (Supporting Recovery with Medications for Addiction Treatment).

Responsibilities

• Conduct initial screenings and assessments and develop psycho-social histories.
• Facilitate individual, group, and family counseling within SUD treatment.
• Determine appropriate Level of Care using LOCADTR and related criteria.
• Develop and monitor treatment/recovery plans with patients and the team.
• Document treatment progress and maintain confidential case records.
• Coordinate care management and link patients to community resources.
• Provide crisis intervention and refer to additional services when needed.
• Advise on job training, housing, and education resources as part of recovery.
