Job summary

NYC Health + Hospitals/Gotham Health is seeking an Addiction Counselor in a NYC care setting. The role involves conducting assessments, counseling patients with SUD, coordinating care, and developing recovery plans within a multidisciplinary team.

You will work in a high-volume ambulatory care network, participate in QA/PI activities, and ensure documentation meets regulatory standards while supporting patient recovery and family engagement.

Qualifications

• Proof of completion of OASAS 'Supporting Recovery with Medications for Addiction Treatment (MAT)' training.
• Valid CASAC Level 1 Certificate OR licensure as a Qualified Health Professional (QHP) with appropriate experience/training.

Responsibilities

• Conducts initial screenings and assessments, and performs ongoing toxicology screenings to identify SUD/COD and formulate history.
• Facilitates individual, group, and family counseling; provides psycho-education and skills training.
• Determines Level of Care using LOCADTR criteria to place patients appropriately.
• Develops and monitors treatment/recovery plans with goals and interventions; documents outcomes.
• Participates in staff meetings and documents services for compliant reimbursement.
• Provides orientation and coordinates with families, expands community resources, and supports discharge planning.
