Job summary

Neighborhood Health is seeking a Patient Navigator to enroll members in TennCare, CoverRx and the Federal Health Insurance Marketplace, and to coordinate ongoing care with providers. This full-time role reports to the Manager of Patient Navigation and involves community engagement to support enrollment and access.

The position requires collaboration with diverse patient populations, strong communication, and the ability to navigate health coverage programs effectively.

Qualifications

• Bachelor's/Associate degree or equivalent experience.
• Three+ years in a clinical setting or similar role.
• Experience with eligibility determination and enrollment processes preferred.

Responsibilities

• Enroll patients in TennCare, CoverRx, and the Federal Marketplace.
• Coordinate ongoing care between patients and providers.
• Represent Neighborhood Health at community events when needed.
• Maintain accurate documentation of eligibility determinations and enrollments.
• Provide information and referrals to patients to maximize coverage and access to care.
