Open Sky Community Services is seeking a Respite Navigator to provide case management support to adults with complex mental health needs in the community. You will be the primary contact for providers across housing, finance, employment, and transportation, coordinating outreach and assessments to build individualized plans.
Qualifications include a High School Diploma/GED and experience in behavioral health; valid driver's license and ability to work independently are required.
Respite is committed to helping people maintain their independence in the community – to live a life that isn’t defined by their mental illness. We meet individuals where they are at; in settings and spaces where they feel safe and comfortable. The program is a short-term group living environment that provides treatment of intermittent crisis and stabilization of adults with are diagnosed with a mental health/substance use disorder. The goal of the program is to work collaboratively with individuals in developing skills that allow them to live independently and successfully in the community. The Respite Navigator provides case management support to adults in the community with complex mental health needs. Many of these individuals need assistance connecting to community resources such as housing, finance, employment, transportation, community providers, etc. The Navigator will act as the primary point of contact for community providers and will conduct outreach, engagement, assessment, and person-centered treatment planning for individuals who are enrolled in the Mobile Respite program.
Pay Range: $22.07-$22.07
