Job summary

Health Partners of Western Ohio in Lima, Ohio seeks a Certified Application Counselor to educate and assist patients and residents with enrollment into affordable health insurance options including Health Insurance Exchange, Medicaid, CHIP, and Medicare.

You will screen uninsured patients, help determine eligibility, guide through the Marketplace applications, and ensure services are accessible with respect to privacy and HIPAA.

Qualifications

• High School Diploma or equivalent required.
• Associates in Health or Human Services or related field preferred.
• Experience with underserved and vulnerable populations preferred.
• Knowledge of state and federally funded public benefits programs preferred.
• Must be able to work independently and travel between sites.

Responsibilities

• Conducts outreach to uninsured or underinsured individuals within the service area.
• Completes CAC training and maintains enrollment expertise.
• Assists with determining eligibility for Medicaid, CHIP, and Marketplace programs.
• Guides patients through a single application for eligibility and financial assistance.
• Preserves privacy and HIPAA compliance in all interactions and documentation.
• Documents all interactions in the designated system and coordinates referrals.
