Responsibilities

• Plan and conduct individual, small and large group activities.
• Assist in communicating with residents, families, staff, and external parties.
• Help develop monthly activity calendars and maintain attendance records.
• Assist in discharge planning and activity care plans.
• Arrange transportation for residents to outings, appointments, or discharge.
• Assist with activity assessments and quarterly progress notes.
• Keep the Activity Department clean, orderly and secure.
