Job Summary

As a member of the multidisciplinary ACCS Integrated Team, the Integrated Team Engagement Partner II provides day‑to‑day support to adults challenged with mental health issues. The role delivers interventions that are clinically informed and evidence‑based, partnering with Persons to work toward personal goals to achieve greater independence and improved quality of life.

Schedules Available

• 7am - 3pm
• 8am - 4pm

Supervisory Responsibilities

At the discretion of the ACCS Clinical Supervisor.

Essential Functions of the Position

• Incorporates evidence‑based practices and philosophies into engagement strategies, treatment planning, and interventions based on each Person’s assessed readiness and expressed interest to support long‑term recovery (e.g., SBIRT, Stages of Change, Motivational Interviewing, Harm Reduction, Seeking Safety, Trauma Recovery and Empowerment Model).
• Participates on the Engagemen Team of assigned Persons as directed by the ACCS Clinical Supervisor.
• Uses recovery‑oriented language and principles in all communication with Persons, colleagues, and collaterals.
• Engages in supportive interventions that teach daily living skills such as community safety practices, travel training, banking and budgeting, cooking, shopping, personal care and hygiene, and care of personal space and belongings.
• Collaborates with Care Coordination Entities and other providers to ensure access to medical, clinical, self‑help, and peer modalities during crises, care transitions, life transitions, and changes in health status.
• Accompanies Persons to medical, dental, psychiatric appointments or leisure activities via public transportation, agency vehicle, or staff car as needed.
• Collaborates with the ACCS Housing Coordinator to meet licensing requirements for GLEs, SIEs, and independent apartments, monitoring deficiencies and developing strategies to support positive tenancy.
• Administers medications in accordance with MAP standards and trains clients in self‑medication procedures.
• Performs other duties as assigned by supervisor.

Requirements for the Position

• Bachelor’s Degree required, preferably in Psychology, Social Work, or a related field.
• High School diploma or GED plus three years of direct care experience with adults with mental health issues or a related population can substitute the degree.
• Strong commitment to person‑centered care and community living for individuals with mental health challenges.
• Flexibility to work evenings and weekends on a scheduled or as‑needed basis.
• Proficient writing and verbal communication skills.
• Excellent personal organization, attention to detail, and time‑management skills.
• Proficiency with word processing and other computer functions using Windows, Word, and Google Apps.
• Department of Public Health certification in medication administration required within 3 months of hire.
• CPR and First Aid certification required within 3 months of hire.
• Physical ability to navigate stairs and assist clients with mobility impairments as needed.
• Use of a personal cellular telephone for work communication.
• Valid driver’s license preferred.
• Bilingual candidates encouraged; pay differential offered for fluency in essential languages.
