Job summary

Jobtailor is seeking a dedicated care coordinator to support youth and families within OhioRISE. The role focuses on coordinating services for children with behavioral health needs, collaborating with families, schools, and community partners to implement care plans and crisis safety strategies.

You will engage in both in-person and virtual meetings, document care activities, and participate in ongoing training to maintain high standards of collaborative care and cultural humility.

Qualifications

  • Experience providing services and/or support to children and families in behavioral health, child welfare, developmental disabilities, juvenile justice, or related public sector services.
  • Hi-Fidelity Wraparound certification preferred (Certification provided at time of employment).
  • Two years in a coordinated supportive services or care coordination role preferred.
  • Experience with autism spectrum disorders and developmental disabilities preferred.
  • Work with families experiencing serious emotional disturbance and cross-system involvement.
  • Strong organizational skills and ability to manage multiple tasks.

Responsibilities

  • Assist in the immediate care coordination needs of youth hospitalized due to a behavioral health condition enrolled in the OhioRISE program.
  • Support families with hospital discharge planning, educating families and partners about OhioRISE, and referrals for follow-up care.
  • Attend in-person and virtual meetings to complete engagement milestones, including crisis safety plans and care plans.
  • Join with family to identify care coordination needs/services in line with standards and program outcomes.
  • Work with families to define cultural factors influencing strengths and family interaction styles.
  • Coordinate family-based services for children, youth, and families in home, school, and community.
  • Engage families with child and family teams to support integration of team members and care plans.
  • Develop partnerships with community resources to meet diverse youth and family needs.
  • Maintain documentation, participate in program evaluation, and complete trainings.

Job description

  • Assist in the immediate care coordination needs of youth hospitalized due to a behavioral health condition enrolled in the OhioRISE program.
  • Support families with the hospital discharge process, educating families and community partners about the OhioRISE program, and making referrals for follow-up care and services.
  • Attend in-person and virtual meetings to complete engagement milestones for the program, including crisis safety plans, assessments, care plans, and scheduling and facilitating Child and Family team meetings until a warm handoff can be made to the youths ongoing care coordinator.
  • Join with family to identify care coordination needs/services in line with service delivery standards and program outcomes to ensure best outcomes for children, youth, and families.
  • Work with families to define cultural factors that influence strengths, functioning, and family interaction styles to ensure ongoing engagement and success in care planning.
  • Identify strengths of children, youth, and families for utilization in care coordination engagement and supporting healthy outcomes.
  • Coordinate family-based-services for children, youth, and families in their home, school, and community.
  • Ensure with family that services identified on care plans are the most appropriate, least restrictive, and meet the safety and treatment needs of the child, youth and family.
  • Engage and build positive relationships with children, youth, and families in coordination with child and family teams to support successful integration of team members and care plans.
  • Develop collaborative and creative partnerships with community resources to meet the diverse needs of youth and families.
  • Maintain necessary documentation, participate in program evaluation, attend team and program planning meetings, cross-systems training, and acquire knowledge of community resources.
  • Remain current with all training requirements including but not limited to High Fidelity Wraparound, MI, Cultural Humility, etc.
Requirements
  • Experience providing services and/or support to children and families connected to behavioral health, child welfare, developmental disabilities, juvenile justice, or a related public sector human services or behavioral healthcare field: three years with a high school diploma or equivalent; or two years with an associate degree or bachelor's degree; or one year with a master's degree or higher
  • Knowledge and experience in Hi-Fidelity Wraparound preferred (Certification provided at time of employment).
  • Two years of experience in a coordinated supportive services or care coordination role preferred.
  • Experience working with people with autism spectrum disorders and developmental disabilities preferred.
  • Experience in one or more of the following areas: family systems, community systems and resources, case management, child and family counseling or therapy, child protection, or, child development.
  • Be culturally humble or responsive with training and experience to manage complex cases
  • Have the qualifications and experience needed to work with children and families who are experiencing serious emotional disturbance (SED), trauma, co-occurring behavioral health disorders and who are engaged with one or more child serving systems (e.g. child welfare, intellectual and developmental disabilities, juvenile justice, education)
  • Excellent organizational skills with the ability to stay focused and prioritize multiple tasks.
  • Experience with multi-need individuals and families.
  • Broad knowledge of community service systems.
  • Willing to participate in and lead cross-systems care coordination.
  • Able to effectively communicate through verbal/written expression.
  • Must be able to operate in an Internet-based, automated office environment.
  • Valid Driver License required.
  • Ability to operate, receive and conduct video calls and/or meetings in an automated office environment is required.
  • Ability to maintain a minimum of 100mbps for high-speed internet connection is required.
