Hill Valley Healthcare is seeking a compassionate Physical Therapist to join our team and deliver high-quality patient care. You will perform PT assessments, develop treatment plans, and provide direct care to patients while documenting in PCC EMR.

Qualifications

• Graduated from an accredited Physical Therapy (PT) college or university.
• Valid Physical Therapy (PT) license for state practicing.
• Conduct Physical Therapy assessments of referred patients providing direct patient care to assess medical conditions, functional capabilities, limitations, and potential for rehabilitation.
• Proficient in EMR documentation (we use PCC but experience in this system is not required).
• CPR Certification.

Responsibilities

• Conduct PT assessments and provide direct patient care.
• Develop and implement treatment plans in collaboration with patients.
• Document patient progress in PCC EMR and coordinate with the care team.
