Responsibilities

• Promotes patient and family centered care in a healing environment.

• Participates in patient safety, quality and regulatory compliance.
• Develops, implements, and documents individual plans of care with defined goals with the interprofessional team.
• Proactively communicates the plan of care across the continuum of care.
