Qualifications

• Board certification eligibility or status required for practice in Arizona.
• Endodontic residency certificate required.
• Graduate from an accredited dental school.

Responsibilities

• Provide endodontic care in a comprehensive dental office.
• Maintain clinical autonomy within a PPO practice.
• Deliver patient-centered endodontic treatment and care coordination.

Position Qualifications

• Board Certified/ Board Eligible
• 1 year of experience preferred
• Endodontic Residency Certificate
• Accredited Dental School Graduate
• Current applicable State Dental License (or the ability to acquire one by time of employment)
• Other licenses/certifications such as CPR, DEA, NPI
