Qualifications

• Performs comprehensive patient assessments across clinical situations.
• Uses clinical judgment to evaluate symptoms and support care decisions with providers.
• Delivers full range of nursing tasks: vitals, meds, vaccines, procedures, patient comfort and safety.
• Provides patient education and self-care guidance.
• Coordinates care plans, communicates results, and arranges referrals.

Responsabilités

• Performs timely and accurate patient assessments across a variety of clinical situations.
• Uses sound clinical judgment to identify patient needs and support care decisions with providers.
• Delivers comprehensive nursing care: rooming patients, vitals, medications, vaccines, procedures, and clinical tasks.
• Responds to patient questions and concerns via phone, portal, and in-person.
• Coordinates care planning, results communication, and referrals to ensure continuity of care.
• Supports follow-up appointments and timely interventions for continuity of care.
