Job summary

Qualifications

• 2+ years of RN experience required.
• RN licensure and BLS certification required.
• Knowledge of EMR documentation and patient care coordination.
• Bachelor degree in Nursing required.

Responsibilities

• Manage patient flow, triage and direct patient care.
• Coordinate communication among front desk, nursing staff and physicians.
• Develop patient outcomes and goals with providers.
• Provide nursing support to optimize outcomes and quality of life.
• Coordinate continuity of care post-discharge and post-ER visits.
