Aria Healthcare is seeking a Licensed Practical Nurse (LPN) to deliver quality nursing care to residents and to supervise day-to-day nursing functions performed by Certified Nursing Assistants.

You will coordinate care with other disciplines within the facility, following Federal and State regulations. This role requires an active LPN license in Wisconsin and a commitment to patient safety and compassionate care.

Qualifications

• Must hold an active Wisconsin LPN license.
• Demonstrates ability to coordinate care with interdisciplinary team.

Responsibilities

• Provide and administer medications as ordered by the physician.
• Review and transcribe orders; document resident information.
• Assist in admission/discharge processes and care planning.
• Complete incident reports and follow up investigations.
• Maintain rapport with disciplines to ensure comprehensive resident care.
• Communicate effectively with residents, families, and staff.
