Job summary

Aria Healthcare in Wisconsin is seeking a Licensed Practical Nurse (LPN) to provide compassionate nursing care to residents and coordinate daily activities with the care team.

The LPN will administer medications, assist in care planning, and participate in ongoing education to support safe, high-quality resident outcomes. Open interviews for nurse positions are held Tuesdays from 11am to 2pm.

Qualifications

• Must have current and active license to practice as a Licensed Practical Nurse (LPN) in Wisconsin.
• Experience in long-term care settings preferred.

Responsibilities

• Prepare and administer medication as ordered by the physician.
• Review medication orders for completeness and accuracy.
• Transcribe physician orders.
• Document resident information and update care teams as needed.
• Assist in Admission/Discharge processes.
• Complete incident reports and follow up investigations.
• Participate in care planning conferences and maintain care documentation.
• Develop rapport with residents, families, and interdisciplinary teams.
