Job summary

Aria Healthcare is seeking a Licensed Practical Nurse (LPN) to provide high-quality nursing care to residents and coordinate the day-to-day nursing activities with CNAs. The LPN will ensure care plans are followed and communicate effectively with physicians, families, and other disciplines.

The role requires a current Wisconsin LPN license and a commitment to teamwork, safety, and continuous education within a supportive facility environment.

Qualifications

• Must have current and active license to practice as a Licensed Practical Nurse (LPN) in Wisconsin.

Responsibilities

• Provide quality nursing care to residents and supervise day-to-day nursing functions performed by CNAs.
• Coordinate all aspects of a resident's care with other disciplines in the facility.
• Document all resident information and notify physician and family as needed.
• Assist in Admissions/Discharge process.
• Complete accident/incident reports and follow up investigations.
• Participate in care conferences and update care plans.
