Job summary

Dedicated caregivers home care services llc is seeking a Community Support Professional to help clients achieve greater independence and quality of life by supporting housing, healthcare, employment, and personal development within the community.

You will build rapport, coordinate referrals, maintain contact per service plans, document encounters within 24 hours, and collaborate with families and providers to implement care plans.

Qualifications

• High school diploma or GED required.
• Bachelor’s degree in Human Services, Social Work, Psychology, or related field preferred.
• Minimum of 2 years of experience in homecare or community-based human services.
• Strong communication, documentation, and organizational skills.

Responsibilities

• Establish and maintain professional rapport with clients and uphold ethical boundaries.
• Connect clients with local resources for housing, healthcare, and social inclusion.
• Maintain regular contact with clients as defined in their service plan.
• Coordinate referrals for medical care, entitlements, transportation, education, employment, legal assistance, and more.
• Assist with money management and obtaining entitlements.
• Develop, implement, and monitor person-centered care plans.
• Enhance independent living skills through instruction and hands-on support.
• Identify barriers to clients’ goals and connect them to services.
• Conduct regular meetings to review progress and discharge readiness.
• Perform routine apartment inspections and assist with cleanliness.
• Build collaborative relationships with families and external providers.
• Facilitate psychoeducational or skill-building groups as needed.
• Attend team meetings, trainings, and supervision.
• Support clients with completing redetermination forms and SNAP applications.
• Submit encounter notes within 24 hours and monthly summaries.
