Job summary

Responsibilities include building rapport, coordinating referrals, supporting money management, and developing care plans from intake to discharge, with regular meetings and progress reviews.

Qualifications

• High school diploma or GED required.
• Minimum 2 years of experience in homecare or community-based human services.
• Strong communication, documentation, and organizational skills.

Responsibilities

• Establish and maintain professional rapport with clients.
• Connect clients with local housing, healthcare, and social resources.
• Maintain regular contact with clients as per service plan.
• Coordinate referrals for medical care, entitlements, transportation, education, employment, legal aid.
• Assist with money management and entitlements.
• Develop and monitor person-centered care plans from intake to discharge.
• Enhance independent living skills through instruction and hands-on support.
• Identify barriers and connect to services.
• Conduct regular progress meetings and review sessions.
• Perform routine apartment inspections and assist with cleanliness.
• Build collaborative relationships with families and external providers.
• Facilitate psychoeducational or skill-building groups.
• Attend team meetings, trainings, supervision, and rounds.
• Assist with redetermination forms and SNAP applications.
• Submit encounter notes within 24 hours and monthly reports by the 21st.
