Job summary

Our Community Our Kids in Plano, TX is seeking an Intake Specialist to manage referrals and coordinate youth placements in licensed facilities. You will review intake information from DFPS and other agencies, handle crisis calls, and serve as a liaison with residential providers.

The role requires a Bachelor's in Social Work or related field, plus strong verbal and written communication, collaboration with families and partner agencies, and adherence to confidentiality and trauma-informed care

Qualifications

Must have excellent verbal and written communication skills.
Ability to work collaboratively with youth, families, collaborating agencies, and staff.
Willingness to learn and apply Trauma-Informed Care principles.

Responsibilities

Receive and review referrals and intake information from DFPS and other agencies.
Coordinate placement of youth in appropriate facilities.
Serve as liaison for agency youth in residential and psychiatric facilities.
Ensure placement adheres to OCOK policies and outcomes.
Maintain up-to-date listing of network providers and licensing capacity.
Participate in meetings with partners and trainings.
Complete intake and placement documentation.
Maintain client confidentiality per agency standards.
