Job summary

COMMUNITY WORKSHOP & TRAINING in Peoria, IL seeks a Vocational Case Manager to develop and implement Individual Service Plans for consumers with disabilities. You will maintain confidential consumer records and collaborate with families, residential programs, and staff to achieve program goals.

The role requires organizing data, providing direct support, and coordinating with workshop crews while pursuing QIDP certification through state-m mandated training.

Qualifications

• Bachelor's degree in rehabilitation, social work, psychology, or related field.
• Ability to formulate individual goals using person-centered planning.
• Valid driver's license and current auto insurance.
• Experience working with individuals with developmental disabilities and mental illness.

Responsibilities

• Implement the Individual Service Plan/Implementation Strategy with consumer, family, and other professionals.
• Maintain consumer records ensuring accuracy, confidentiality, and timely sharing with cooperating agencies.
• Provide individual support to consumers as needed.
• Analyze program data sheets and document monthly progress notes.
• Serve as liaison with crew chiefs to document individual functioning in the workshop.
• Complete State of Illinois mandated training to maintain QIDP certification.
