Job summary

Qualifications

• Must have a High School Diploma or GED.
• Must be at least 18 years old.
• Must be able to communicate effectively both verbally and in writing.
• Knowledge of Person-Centered Planning and CQL concepts is preferred.
• Ability to lift up to 50 pounds and navigate various work environments.

Responsibilities

• Provide individualized instruction, training, and coaching for community employment.
• Monitor schedules, attendance, and job performance with employers.
• Assist with transport to and from community sites as needed.
• Support transitions toward integrated community employment and natural supports.
• Maintain regular communication with employers and IDT members about progress.
