Job summary

LiveWest is recruiting a Support Worker for its New Horizons scheme covering East Cornwall. The role supports customers with complex needs, helping them build independence within their communities. The position is part‑time (22.2 hours/week) with Monday–Friday pattern and one weekend every six weeks.

Qualifications

• Experience of supporting people with a range of needs and backgrounds.
• Ability to build effective, professional working relationships.
• Confidence working independently, including lone working.
• Ability to make informed decisions and take accountability for actions.
• Experience of maintaining accurate records and action plans.
• Willingness to work flexibly to meet service and customer needs.

Responsibilities

• Delivering high‑quality, person‑centred support tailored to individual needs.
• Building positive and professional relationships with customers, colleagues and partner agencies.
• Supporting customers to identify goals and develop personal support plans.
• Promoting independence, inclusion and participation to reduce social isolation.
• Completing risk assessments and managing risk in line with policies and procedures.
• Maintaining accurate records and contributing to service performance targets.
• Working safely, including lone working, while maintaining clear professional boundaries.
