Job summary
Hampton-Newport News Community Services Board is seeking a Support Coordinator for adults with intellectual and developmental disabilities (IDD). You will help individuals access services and supports to live as independently as possible in the community.
The role requires a Bachelor's degree in Human Services and at least one year of case management experience. The annual salary is $50,604 and typical schedule is Monday to Friday, 8:30 am to 5:00 pm.
Qualifications
- Bachelor's degree in Human Services, Social Work, Psychology, Counseling, or related field.
- One year of experience providing case management or support coordination services to adults with developmental disabilities.
Responsibilities
- Develop and monitor individualized service plans based on each person's needs, strengths, preferences, and goals.
- Coordinate medical, behavioral health, residential, employment, transportation, and community-based services.
- Advocate for individuals and provide ongoing support, crisis intervention, and assistance with daily living needs.
- Collaborate with individuals, families, healthcare professionals, and service providers to ensure appropriate and effective supports.
- Promote independence, self-determination, community inclusion, and quality of life while maintaining accurate documentation.
