Job summary

Health Care Resource Centers in Hartford seeks a compassionate Substance Abuse Counselor to support patients through medically assisted programs and teach coping strategies.

You will conduct group and individual sessions, help develop treatment plans, and monitor progress while collaborating with doctors, nurses and therapists for integrated care.

Qualifications include CADC/CAC in CT; LCSW/LICSW, LMHC or LADC preferred; bilingual helpful; background check and drug screen required.

Qualifications

• Must hold a CADC or CAC in CT.
• Preferred: LCSW/LICSW, LMHC and/or LADC.
• Demonstrated counseling experience.
• Previous experience working with chemically dependent individuals.
• Bi-Lingual preferred.
• Satisfactory criminal background check and drug screen.

Responsibilities

• Conducts initial assessment and evaluation interviews for service eligibility.
• Facilitates and reviews initial treatment plans with each patient.
• Completes Bio-psychosocial assessment.
• Provide individual and group counseling; direct service hours; coordinate all services.
• Develops and participates in individual or group patient teaching projects.
• Responds to patient grievances and complaints as needed.
• Other related duties as determined by supervisor.
