Job summary

Qualifications

• Active Speech Language Pathologist licensure in the state of assignment is required.
• CCC-SLP certification preferred and may be required by state/contract.
• Complies with all relevant professional standards of practice.
• Current CPR if applicable.
• TB questionnaire or chest x-ray if applicable.
• Current Health certificate (per contract or state regulation).
• Must meet all federal, state and local requirements.
• Must be at least 18 years of age.

Responsibilities

• Performs student evaluations.
• Delivers and documents therapy services per plan of care.
• Follows physician orders and plan of care for each student.
