Responsibilities

• Assist in developing individualized, person-centered service and support plans and help implement.
• Provide support with daily living activities, skill development, and personal goal achievement.
• Assist individuals with transportation and community resources as needed.
• Maintain a safe, clean, and supportive environment and complete required documentation.
