Job summary

AuthoraCare Collective is seeking a Physical Therapist Assistant (PTA) Case Manager for Home Health in Greensboro, NC. This full-time role runs Monday–Friday, 8:00am–5:00pm, delivering services to restore function and improve mobility under the guidance of the Physical Therapist.

You will work with an interdisciplinary team, maintain treatment plans, conduct home visits, and support patients and families with education and resources.

Qualifications

• Associate degree in Physical Therapy from an APTA
• Current and unrestricted PTA licensure
• CPR certification is required
• One year PTA experience in home health or facility setting preferred

Responsibilities

• Directs and coordinates care per the patient’s Plan of Treatment
• Assesses patient/family needs and implements the interdisciplinary Plan of Treatment
• Works with IDT to identify goals and maintain the Plan of Care
• Establishes therapeutic relationships and provides patient education
• Conducts home visits and communicates Plan of Care changes
• Follows infection control and exposure policies

Education
