Eagleview Health and Rehab is seeking a licensed Physical Therapist to join our care-focused team. You will assess residents, develop individualized treatment plans, and work as part of an interdisciplinary group to support optimal recovery.

Qualifications

• Current and active Physical Therapist license in the state of operation.
• Prefer experience/knowledge in older adult rehabilitation.

Responsibilities

• Provides and directs resident rehabilitation care per accepted practices and standards.
• Evaluates residents and establishes effective treatment plans.
• Coordinates supervision of treatment plans with staff and other disciplines.
• Screens residents according to schedules and practices.
• Provides feedback to supervisor on clinical and rehabilitation issues.
• Participates in center processes and services.
• Manages treatment and related activities and documents appropriately.
• Participates in in-service training programs.
